Before You Start Your Application

Please note that if you use the ‘Save and Continue Later’ option, your progress will be saved for 60 days, but any documents you uploaded will not be retained. We therefore recommend waiting to upload your attachments until you have completed all other fields and are ready to submit your application.

For your convenience, we also provide a sample PDF form as a reference to help you prepare your answers before starting the online application. This document is for preparation only and must not be sent as your application. All applications must be completed and submitted through the online form.

• Make sure that all sections are completed as incomplete applications cannot be processed.
• The IUHPE Assessment Committee can ask you to supply additional information and/or seek verification of all information given in this application form so please ensure that you have collected and retained all relevant information (e.g. awards, certificates, etc.).

Self-Assessment of Competence

You should read through the complete IUHPE Competencies and Professional Standards and identify relevant examples of your work experience that demonstrate how you meet the Performance Criteria for each domain. These examples should explain concisely how your experience relates to the performance criteria and be easily verified if necessary.

In the following section, for each domain there is a list of the competencies, knowledge, skills and Performance Criteria necessary for that area of Health Promotion practice.

For each domain you must provide a brief summary (max. 1300 characters) of Health Promotion work that you have done that demonstrates the Performance Criteria for that domain. At least 50% of your examples MUST describe work done in the last five years.

You may use examples from different areas of your work to demonstrate the Performance Criteria for one domain or use the same work example to illustrate Performance Criteria across more than one. However, it is recommended that you draw on different areas of your experience as far as possible.

Please note: You may be asked to provide evidence of all experience, activities, education and training that you include in your application at any point in the application process or at any point during the period you are registered.

Thinking Ahead: It is suggested that you use your reflection in completing this form to identify learning needs which you can address in Continuous Professional Development (CPD) activities over the next three years, as evidence of CPD is required for re-registration.
